3. Legal Nurse Consultant (LNC)

·         Legal Nurse Consultant (LNC) experience has included help initially with the medical research; analysis of SOC/SOP, any breaches of the elements, if there indeed was causation leading to damages and if the damages were significant enough to legally pursue. One case is still in discovery; another, he suggested dropping due to comparative negligence [it was dropped]; another case he was deposed several months ago [defense attorneys wanted an ER Expert, but speaking for the plaintiff] it is scheduled for trial in the fall some time. He has been asked for his Professional opinion as to ER procedures/SOC from an attorney in Wisconsin; He has helped with locating Federal Experts; He worked side by side with the Risk Management in 2 double-blinded drug studies in treatment of acute Stroke, one was a clot buster [thrombolytic agent] another was an anti platelet inhibitor with the glyco proteins llb/llla in the prevention of clot formation. These studies are over now and the drugs are on the market now; he coordinated this study at Baptist Hospital in Nashville. He worked with the hospital attorneys, risk management, case management. Every 3 months, a consultant for the Integrated Neuroscience Consortium would visit each site to see if the site was in compliance with all the Rules, Policies of the Pharmaceutical Co., and so on. When his site was visited, he was offered a position as a Regional Consultant for new sites in setting up their studies. He turned it down even though it was double his salary because it was ALL travel.  These studies required a daily assessment of the patients that were randomized, follow them through each protocol of the study which may have 6-10 protocols from 2 weeks to 6 months, requiring lab analysis, Cardiogram analysis, Urinalysis, Radiographic Imaging [Computerized Tomography- CT scans, MRI's, etc.). This coordination was heavy in critical care assessment, noting and treating adverse events mild-severe-to Sentinel events, with proper reports etc.

·         Experience handling cases involving product liability -His first 3 cases involved personal injury due to negligence, one was dropped at his suggestion due to comparative negligence; but this case still had some negligent and med mal issues easily arguable due to SOC violations, gross poor judgments; 3rd case was negligent in assessing patient and timely and properly treating with patient demise as the outcome. Two of the cases were so egregious that he could not believe they did not settle already. Without breaking privilege, one had to do with the handling of an obese patient, standing, while still anesthetized from the C-Section and stating "I can't feel my feet" this was a 300# patient [suffering fractured ankle in house]; another case was patient suffered fluid volume overload even when the patient advised of her kidney failure, CHF, liver problems, and no documentation of Urinary function- UOP- while getting too much IVF's until she crashed...also leukocytosis without medical treatment or Medical attention to the lack of no urine output [UOP].

·         Strong cardiology background. He has been certified in Advanced Cardiac Life Support since 1983--this is before the American Heart Assoc. had any type of algorithms. He had to be able to diagnose via heart monitors in the ambulance with Davidson Co. Emergency Medical Services in less than 5 sec and know the treatments 1st and 2nd line of drug choices for each cardiac arrhythmia. That was how he was tested in the late 1070's with Univ of Tn Critical Care Paramedic School--5 sec to look, one min to treat and reassess, he has spent 28 years in critical care, Advance Life Support, worked with 'fresh hearts' 12 hours post CAB, was in charge of that critical care unit, 20 beds, each half [10 beds] you were in charge of all the nurses and their care. He has been an Instructor for the Am Heart Assoc. in teaching Adult and Pediatric advanced Life Support. He has worked constantly for these years with acute MI's, as the technology changes and the medical care. Streptokinase, Altepase, and the other clot busters. The bulk of his practice as a Board Certified Emergency RN has been with cardiac patients. Acute and chronic.   His cardiac specialty is his Critical Care training and practice for 28 years, his certifications that are renewed every 2 years since 1983, his Board Certification Emergency RN [CEN} Treating acute MI's on a weekly basis; He has had times where as a Supervisor and a staff nurse that was running 3 cardiac arrest all at the same time. Every shift at work we constantly and consistently treat acute cardiac problems from arrests to cardiomyopathies, Congestive Heart Failure, Pulmonary Edema....any Emergent Cardiac situation one could imagine. He used and still uses thrombolytics for MI's in progress; then on the Stroke side, using the same thrombolytics for Strokes as used in MI's.  At the present, treating acute Strokes due to clots is at the same point we were at 10 years ago with using the 'clot busters' with heart attacks [MI's]. The product liability would best be described with the experience and contact with patients as coordinator of the Acute Stroke Program..it was blinded [2 studies] tPA, Altepase, as mentioned above. The way product liability would come in to the factor would be in critical analysis of the patient as they were given the drug, or placebo, and documenting and treating any adverse event no matter the severity.
